GENERAL PROVISIONS
(1) The organizer of the event entitled: “11 FORUM OF DEVELOPMENT INITIATION. WORKERS’ VOLUNTEER FORUM”, hereinafter referred to as the “Forum”, is the UP Foundation for Initiation of Development with its registered office in Gdansk, 11A Jaskowa Dolina Street, 1A, 80-252 Gdansk. NIP: 9571059793, hereinafter referred to as the “Organizer”.
2 The forum is held in a hybrid formula:
a) Online – on youtube, on the foundation’s channel
b) Olivia Centre, Olivia Sky Club, 80-309 Gdańsk
(c)In the workshop rooms at the Olivia Centre:
1. Olivia Gate, GFKM headquarters, 472 Grunwaldzka Avenue, 80-309 Gdańsk
2. Olivia Gate, room O4 Flow, 1st floor, 472 Grunwaldzka Avenue, 80-309 Gdańsk
3. Olivia Star, Las/Park room, 32nd floor, 472C Grunwaldzka Avenue, 80-309 Gdańsk
(3) Participation in the Forum for those who have been qualified by the Organizer to participate will be free of charge.
4 The number of seats at the Forum is limited.
5 The participant may participate:
(a) throughout the event,
(b) only in the workshop, to choose from three options.
This depends on the place of registration, number of seats and attendance lists.
6 Taking part in the Forum implies reading the Regulations and accepting their terms and conditions.
7 Persons participating in the Forum are hereinafter referred to as “Participants “. Participant – natural persons who are at least 18 years of age and have full legal capacity, legal persons and organizational units without legal personality, but able to acquire rights and incur obligations on their own behalf, who, with the help of konfeo, correctly registered on the Forum by the date indicated in the registration service.
8 The Rules of Procedure are addressed to each person participating in the Forum during its duration.
Persons participating in the Forum, during its duration, are obliged to strictly comply with the provisions contained herein. Each Participant is obliged to behave with respect for other participants and, in particular, to observe the provisions of the Forum Regulations and the law, as well as to comply with the instructions of the Organizer.
9 The purpose of the Regulations is to ensure the safety of the Forum by defining the rules of behavior of Participants regulate their rights and obligations and organizational rules.
(10) By “Registration” is meant the consent of the Organizer to participate in the Forum. Successful online and in-store registration at Olivia Centre, Olivia Sky Club, 80-309 Gdansk, also means that the Participant has completed and signed the Participation Statement.
§2
TERMS AND CONDITIONS FOR PARTICIPATION
(1) Registration for participation in the Forum runs from October 13, 2022 until the pool of seats is exhausted, but no longer than November 04, 2022. The organizer reserves the right to terminate (or extend) the registration early without giving any reason.
(2) Participation registration is done online at the website (konfeo).
(4) The scope of konfeo includes the service of storing in the database of registration, the User, as well as handling of this registration by the Organizer.
(5) In order to correctly register in konfeo, the Participant is required to complete all the indicated fields.
6 The user guarantees that all data submitted for registration are correct.
(7) By completing the registration, the Participant submits a statement that he/she has read and accepts the contents of these Regulations and agrees to the processing of personal data for purposes related to the implementation of the event.
(8) After completing and confirming the accuracy of the data in the registration form, a message will be sent to the e-mail address provided in it, which is a confirmation of receipt of the application for participation in the Forum. You may also need confirmation from the organizer.
§3
ORGANIZATIONAL RULES
(1) Participants are obliged to comply with the instructions of the Organizer.
(2) Participants shall behave in a manner that does not endanger the safety of others, including in particular the security of data and image of other participants, and in particular comply with the provisions of these regulations.
(3) Participants are obliged to observe safety rules, health and safety and fire regulations, regulations on. COVID 19 in effect on the premises where the Forum is held.
(4) The organizer is not responsible for possible infection of participants with COVID 19.
(3) By participating in the Forum, the Participant consents to the use by the Organizer of the image recorded in connection with the implementation of the Forum, and transfers to the Organizer in the scope of unlimited time and territory all rights to use and dispose of the image of the Participant and his recordings (photographic, audiovisual, sound) recorded in connection with the Forum. The image of the Participant, which was in the aforementioned. way recorded and used shall not be entitled to any (including financial) claims on this account. The Organizer reserves the right to sublicense the use of the captured image of the Participant to official partners and sponsors.
(4) Participants are not allowed to record and make public the image of other Forum participants.
§4
PERSONAL DATA
(1) The administrator of the personal data is the UP Foundation Initiation of Development, based in Gdańsk, ul. Jaskowa Dolina 11A lok. 1A, 80-252 Gdańsk (hereinafter referred to as “Administrator”) entered in the register of the National Court Register by the District Court Gdańsk- North in Gdańsk VII Economic Department of the National Court Register under the number 0000393420, NIP: 9571059793 REGON: 221509149. (2) The Participant’s personal data contained in the Forum participation application form, collected by the Organizer, shall be processed for the purpose of conducting the Forum in accordance with the provisions of the Regulations and in accordance with the wording of separate agreements binding the Organizer with the supporting entities, i.e., the Forum. partners, sponsors.
(3) The participant has the right to request from the Administrator access to the personal data provided, to correct it, as well as to delete it or restrict its processing, as well as the right to object to the processing of the data, and the right to revoke the previously expressed consent to the processing of the data at any time. Revocation of consent will not affect the compatibility of the processing of the Participant’s data carried out on the basis of consent given before its revocation.
(4) The participant has the right to file a complaint with the President of the Office for Personal Data Protection.
(5) Provision of personal data is voluntary, however, it is a condition for participation in the Forum and fulfillment of legal requirements. Failure to provide this personal data will result in the inability to participate in the event, fulfill the obligations to which the Organizer is subject, as well as fulfill any complaints. Data for marketing purposes are provided voluntarily
(6) Participation in the event is at the same time the Participant’s consent to the Organizer’s processing of health data (only for the proper implementation of the procedures described above) within the meaning of Art. 9 paragraph. 2 letters a, c, h and the Regulation of the European Parliament and of the Council (EU) 2016/679 of April 27, 2016. on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46/EC (General Data Protection Regulation OJEU 2016.119.1).
§5
CANCELLATION OF PARTICIPATION IN THE FORUM
A participant in the Forum may resign from participation. Cancellations can be made electronically by sending a message from the email address provided when registering for participation in the Conference, to the email address: k ontakt@upfoundation.pl
§6
ADVERTISEMENTS
(1) A participant may file a complaint via email to the email address:
kontakt@upfoundation.pl, as well as by mail to the Organizer’s registered address.
(2) Complaints may be submitted no later than 14 days after the Forum. If a complaint is submitted by mail, the postmarked date will determine the deadline.
(3) The complaint should contain at least the following data:
(a) indication of the Forum to which the complaint relates;
b) designation of the Participant – name, mailing address, e-mail address, contact phone,
(c) the subject of the complaint,
(d) indication of the facts justifying the complaint.
(4) The organizer shall consider the complaint within 14 days of receipt.
§8
FINAL PROVISIONS
(1) Completion and submission of the online application form (konfeo) implies acceptance of these Regulations.
(2) The organizer reserves the right to change the rules and program of the event, at any time without giving any reason, which will be announced at www.forumrozwoju.org.pl. Participants are not entitled to make claims on this account.
(3) These Regulations are available on the organizer’s website: www.forumrozwoju.org.pl.

(5) The Organizer is not responsible for the consequences of force majeure and interruption of water supply, Internet connection, electricity as a result of which the Forum will be interrupted, which is beyond the control of the Organizer.
(6) In matters not regulated in the Regulations, decisions are made by the Organizer and the relevant provisions of the Law apply.
(7) The Rules of Procedure shall come into force on the date of commencement of registration on the Forum.
